New Patients
We want to make sure your appointment is quick, easy, and enjoyable — that’s why we encourage patients to fill out their forms online ahead of their appointment. Our welcome and patient history forms are located below. If your internet is not working or the forms are giving you trouble, please arrive 10 minutes early to complete the paperwork before your scheduled appointment and bring a list of your current medications with you.
Once you arrive, please provide any vision or medical insurance cards that you may have, including your existing prescription eyewear. This includes computer, reading, or hobby-specific eyeglasses. We look forward to your visit, and please contact our team if you have any questions!
